PATIENT FUSION

In 2012, we adopted our Electronic Health Record (EHR) system, Practice Fusion. Today, Practice Fusion has evolved and created its daughter site, Patient Fusion, which allows patients to connect directly to our office to view diagnoses, immunizations, and prescriptions; track medical record updates in real time; send secure messages to a doctor; and view future and previous appointments with the office. 

To setup a Patient Fusion account, simply ask a the nursing staff or front desk at any time before, during, or after your visit.  If you already have an account, you can login by clicking the link at the top of the page.

New Patients

Transfer to our Office

We are pleased you have selected Children's Choice Pediatrics for your child's health and look forward to meeting you.  To transfer your child's records, please have your previous provider send the health records:

  • Digitally— send or bring a copy of the records (PDF format) to our office via USB Flash Drive, External Hard Drive, or CD/DVD.
  • Hard Copy— send or bring a copy of the paper records.
  • Fax— have your previous provider send the records via fax.  Please complete this cover sheet for your provider.

HELP WITH PATIENT FUSION

If you have any questions regarding Patient Fusion, please call our office during normal business hours.  Unfortunately, we are not able to grant initial access to Patient Fusion over the phone— that will need to be done in office.  Some issues may require the use of our in-house IT department.  If you need to contact our technical support team, complete the form below, and we will respond within 2-3 business days.  

Note:  Please do not use this form to ask for/ seek medical advice.  Dial 911 if you believe you are having a medical emergency.  Do not include any health information/ protected health identifiers in your message.  We only offer technical support for patients of our practice, Children's Choice Pediatrics in Stow, Ohio.  If you are a Patient Fusion user from a different medical practice, we are unable to assist you with Patient Fusion.  You must contact your physician's office for help.
